An individual appointment must be made for each person being tested.

 

We are located at 644 West Putnam Avenue, Suite 203, Greenwich, CT.

 If you are symptomatic or have been exposed to COVID-19: 
A nurse will administer your test(s) while you remain inside your vehicle.   
You must go to the lower level of the indoor parking garage (Parking Level 2) when you arrive.  Please call/ text us 203-987-5796 when you are close to arrival —  there is no cell service in the garage.

 

We are an out of network provider—meaning payment is required when booking your appointment.  However, claim forms are available for submission to your insurance company for reimbursement. Please bring a copy of the front and back of your insurance card to your appointment.
